Self-Funded Medical and Dental Services

Association Member Trust provides self-funded medical and dental services to our participants. We use the services of Delta Dental of New Jersey and Horizon Blue Cross Blue Shield, which provide claims administration and access to provider networks.

Horizon Blue Cross Blue Shield of New Jersey one of the largest provider networks in New Jersey, provides Association Member Trust (AMT) Health Plan participants access to Horizon’s PPO and Direct Access Networks. The Direct Access network contracts with approximately 22,000 providers in New Jersey while the PPO network includes approximately 25,000 providers.

Association Member Trust also participates in the national Blue Cross Blue Shield BlueCard Program. BlueCard covers our participants from coast to coast with over 600,000 physicians and 6,000 hospitals participating in the national network of Blue Cross Blue Shield plans.

Association Member Trust uses the services of Delta Dental to administer its dental plans. The Delta Dental Plan of New Jersey offers one of the largest Dental provider networks in New Jersey, providing Association Member Trust Dental Plan participants access to Delta’s Premier and Preferred Networks.

Delta Dental of New Jersey, first started offering dental programs in 1969. 50 years later, Delta Dental of New Jersey now provides or administers dental coverage to more than 1.7 million people in New Jersey and Connecticut. Delta Dental of New Jersey is a member of the national Delta Dental Plans Association which provides national dental coverage to more than 75 million people in more than 139,000 groups.

Delta Dental of New Jersey's mission is to promote oral health to the greatest number of people by providing accessible dental benefit programs of the highest quality, service, and value.

Association Member Trust also offers group term life insurance plans to participating employees through the UniCare Life and Health Insurance Company.

The Group Life/Accidental Death & Dismemberment program is underwritten by the UniCare Life & Health Insurance Company.

LIFE INSURANCE

The full amount of this insurance is payable to your named beneficiary in the event of your death anywhere in the world, from any cause, regardless of any other insurance you may have at the time.

ACCIDENTAL DEATH & DISMEMBERMENT

The full amount of this insurance is payable to your beneficiary in addition to the Life Insurance if the death is caused by accidental means. In addition, cash benefits are payable in the event of accidental loss of limb or eyesight.

BENEFITS AVAILABLE

All Firms

  • A flat amount for all full time employees $15,000, $20,000 or $50,000
  • One (1) times each full time employee's salary rounded to the next higher $1,000 to a maximum of $50,000

Firm with ten lives or more

  • One and a half times each full time employee's salary rounded to the next higher $1,000 to a maximum of $75,000
  • Two times each employee's salary rounded to the next higher $1,000 to a maximum of $100,000

Note: All amounts are subject to reduction starting at age 65
Age 65 reduces 35%; age 70 reduces 50%; age 75 flat $2,000; retiree's flat $1,000. Coverage amounts of $50,000 or more are subject to underwriting approval.

Association Member Trust uses Prime Therapeutics as its pharmacy benefit manager. Prime manages coverage for medicine to health insurers, employers, seniors on Medicare, and individuals. Prime Therapeutics purpose is to help people get the medicine they need to feel better and live well.

Formed in 1998 by two Blue Cross and Blue Shield plans, Prime Therapeutics has grown steadily and now partners with 18 Blue Plan owner clients. This unique ownership structure makes it possible for us to remain absolutely committed to the health of the members we serve.
